CLL mostly affects the elderly. Optimum management requires evaluation of disease characteristics, comorbidities, and geriatric impairments. Based on these assessments, elderly patients can be divided into those fit for standard intensive therapies, those in whom such therapies need to be adapted, and those best suited to supportive care. Novel agents greatly increase available options.
