Before implicating worse local control due to lower doses used for large cavities, one must consider that smaller tumours have better outcomes with surgical resection alone than do larger tumours.2 In N107C/CEC·3, surgical bed control was better with whole brain radiotherapy (WBRT) than with stereotactic radiosurgery (SRS) for surgical cavities measuring 3 cm or less (p=0·0005), but the difference in local control was actually not significantly different for surgical cavities larger than 3 cm (p=0·24).
