Based on the observation of 50 patients, our clinical prospective study (Goerig et al)1 provided strong evidence for frequent and therapeutically reversible cytomegalovirus (CMV) encephalopathy during radio(chemo)therapy of the brain in seropositive patients with high-grade glioma and in patients with brain metastases. Demonstrating neurological signs of encephalopathy concomitant with appearance or increase of CMV DNA in whole blood and symptomatic improvement accompanied by CMV DNA decline upon antiviral treatment indicated CMV reactivation.
